The Centers for Medicare & Medicaid Services (CMS) Ratings System
The Centers for Medicare & Medicaid Services (CMS), a federal agency within the United States Department of Health and Human Services, has created a five star quality rating system for nursing homes. Facilities are reviewed on a 1 to 5 star rating scale with a 1 indicating below average and a 5 indicating above average quality. These ratings for nursing homes across the United States can be found on the federal government website at Care Compare on Medicare.gov.
How the CMS Rates Facilities
There is one overall rating for each nursing home and a separate rating for each of the following:
- Health Inspections. This includes the three most recent inspections and investigations due to complaints. The inspection team investigating complaints, facility reported incidents, and any deficiencies cited will determine whether a nursing home has met the minimum quality requirements from Medicaid and Medicare to ensure resident life safety code is adequate. The most recent findings are weighted over prior years.
- Staffing. This rating includes information about nursing and administrative staff and the average number of hours of care that are provided to each nursing home resident. This rating takes into account different levels of care provided by a home. Nursing homes providing care for individuals with more severe needs would be expected to have more nursing staff to ensure adequate resident health.
- Quality Measures. The quality measure (QMs) includes information on how well nursing homes are caring for residents’ physical and clinical needs. This is based on 15 different physical and clinical measures.
CMS also has a helpful guide which can prove useful in your evaluation and comparison of nursing homes.

New York State Department of Health Nursing Home Profiles
The New York State Department of Health also provides nursing home ratings intended to allow patients and their families on Long Island and throughout New York to easily determine the performance of NY nursing homes. They include an overall rating as well as ratings in five domains of care including:
- Preventative care
- Quality of care
- Quality of Life
- Resident Safety
- Resident Status
New York’s site allows you to view nursing homes based upon location, which can aid in finding a facility for a loved one close to home.
New York State Domains of Care
These domains encompass 24 different quality measures. Like the CMS ratings, a 1 star indicates the lowest performance and 5 stars indicates the highest performance.
The overall rating indicates the average performance of the nursing home for all domains of care. The domain ratings indicate the level of performance of the nursing home for all quality measures grouped under that domain of care.
The methodology and data reporting periods for the New York Nursing Home Domain Rating data differs from that of the CMS (United States Government) rating system.
New York Nursing Home Complaint Form
New York State also has a Nursing Home Complaint Form on the Department of Health (DOH) website which allows families to make complaints regarding care directly to the DOH. Families can also contact the DOH directly by calling the Nursing Home Complaint Hotline at 1-888-201-4563.

How Can a Long Island Nursing Home Abuse Lawyers Help in Cases of Neglect?
If you have a loved one who is currently in a nursing home or skilled nursing facility, it’s important to be aware of the potential for abuse and neglect. Unfortunately, nursing home abuse is all too common, and can take many different forms, such as physical abuse, neglect, emotional abuse, sexual abuse, and financial exploitation.

Some Long Island facilities may also be inadequately equipped to deal with residents with conditions such as Alzheimer’s disease or dementia, putting them at further risk. A common sign of neglect is seen with skin integrity breakdown, which often results from inadequate care planning and a failure to implement skin protection protocols.
